Immigration Medical Examination:

Dr. Reid is a designated Civil Surgeon authorized to perform Form I-485 medical examinations for the United States Citizenship and Immigration Services (CIS) and other required CIS medical examinations.

The Form I-485 examination is done in two visits, scheduled 48-72 hours apart. The cost of Dr. Reid’s services during the two visits must be paid in cash at the time of the first visit. There may be extra charges billed directly to you for required x-rays or laboratory testing.

Schedule the first visit by calling the office and requesting a one hour appointment for an immigration physical examination. You will need to bring the following:

  • Official photo ID (usually your passport)
  • Immunization records

If you cannot document that you have received each of the required immunizations, you will be either vaccinated or tested to demonstrate that you are immune.

The second visit is brief and will be scheduled at the time of your first visit.

Cost:

Our office cost of the physical must be paid in cash at the time of your first visit. The cost of blood tests, immunizations and x-rays will be billed to you separately.

Dr Reid is authorized to perform examinations for CIS form I-693 and DS-2053 and the associated worksheets DS-3024, DS-3025, DS-3026.
